Findings: Witness 1 (W1) has a known history of physical aggression towards residents. On or about January 15, 2023, AV and W1 had a near-physical confrontation in the common area of the facility. W1 was sitting at a table in the dining room when AV wheeled past them in a wheelchair. W1 said, “Don’t run me over.” AV stated they hadn’t. W1 raised their hand in a fist as if they were going to hit AV. The facility failed to provide a safe environment, which is a violation of resident rights, is neglect of care and constitutes abuse.
Sanction: RCFCP23-00419 $500.00 fine assessed
