Findings: The Alleged Victim (AV) has a history of wandering, an elopement risk, and identified as a high fall risk. According to an investigation, on or about February 10, 2023, AV eloped from the facility after a visitor entered the building. The AV was found on the ground by a pedestrian who contacted EMT’s. EMT’s found the AV bleeding, notified facility, and took AV to the hospital. AV had a laceration to the scalp that required staples, a head injury, lip laceration, contusion of the face and a closed fracture of a nasal bone. The facility failed to provide a safe environment, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP23-00712 $375.00 fine assessed
