Findings: The facility failed to provide a safe environment. According to an investigation, Witness 1 (W1) has a history of behaviors and resident-to-resident altercations. On or about February 9, 2023, W1 got upset when the Alleged Victim (AV) was sitting in a chair that W1 preferred to sit in. As a result, W1 yelled at AV, grabbing his/her arm and striking him/her in the face. The facility's failure is a violation of resident rights, is neglect of care and constitutes abuse.
Sanction: RCFCP23-01341 $450.00 fine assessed
