Findings: The facility failed to appropriately care plan or implement interventions according to the Alleged Victim (AV)'s needs, relating to falls. According to an investigation, AV experienced approximately twelve falls between approximately February 11, 2022, and February 10, 2023, some resulting in injury to include left shoulder injury, upper-mid vertebra abrasion, Skin tear on h/h left forearm. There were approximately four (4) falls where no additional interventions or care plan updates were made. On or about February 11, 2023, AV had another fall resulting in a fractured pelvis. The facility’s failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: ALFCP23-00394 $1500.00 fine assessed
