Findings: On or about February 15, 2023, Alleged Perpetrator 2 (AP2) administered medication to Alleged Victim (AV) that he/she has a documented allergy to which caused AV suffered unreasonable discomfort. Based on facility documentation and interviews, it was determined that AP2 failed to follow facility medication procedures and failed to follow AV's physician orders. AP2's actions is considered neglect of care which constitutes abuse. The facility failed to provide a safe medication administration system which is a violation of Oregon Administrative Rules.
