Findings: Alleged Victim (AV) has a history of exit seeking and has eloped the building prior to this event, putting AV is risk of harm. On or about February 15, 2023, AV was found standing outside having exited through a door leading outdoors behind the kitchen at the loading area. This occurred since AV has a tendency to stand near doorways and had been known to push staff out of the way in order to leave the facility premises. AV’s care plan does not indicate any interventions to prevent AV from exit seeking, which is a violation of resident rights, is neglect of care and constitutes abuse.
