Findings: Witness #1 (W1) is known to have aggressive behaviors with staff and residents. Between December 2022 and February 2023, W1 was documented as having aggressive behaviors with both staff and residents at least 10 times. On or about February 14, 2023, W1 pushed the Alleged Victim (AV) down. When AV stood up after being pushed down, then sat down, AV yelped. W1 had been aggressive with AV in the past, and staff were directed to keep AV and W1 apart. The facility's failure to follow the care plan placed AV at risk for harm. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP24-01013 $500.00 fine assessed
