Findings: Alleged Victim (AV) relies on the facility for a safe environment. Witness 1 (W1) and AV have history of physical resident-to-resident altercation were W1 struck AV causing a bruise. An intervention was placed for staff to cue residents to not walk so close to others. On or about February 15, 2023, AV tried taking a jacket from W1, W1 struck AV in the face, resulting in AV having a cut on h/h lip and a red nose. The facility failed to appropriately adjust the service plan for W1 to prevent physical altercations with AV, The Facility failed to provide a safe environment for AV, which is a violation of resident rights, is neglect of care and constitutes abuse.
