Findings: The facility failed to provide appropriate services according to the Alleged Victim's (AV) needs. On or about February 17, 2023, AV experienced a fall and push h/h call button at approximately 6:18AM. AV waited for approximately three hours, then called 911 for assistance at approximately 9:34AM. The failure resulted in AV experiencing pain and unreasonable discomfort, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
