Findings: According to the documentation, the facility failed to follow the care plan by ensuring the bed cane was put into place when the Alleged Victim (AV) went to bed. The failure resulted in the AV trying to get up during the night causing a fall and the AV becoming unresponsive, needing medical evaluation at the hospital and was diagnosed with a fractured nose, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
