Findings: The Alleged Victim (AV) has a known hesitation to call staff when needing assistance and a history of falls. The facility failed to provide appropriate services according to Alleged Victim’s needs, number of falls with lack of appropriate interventions. Based on an investigation, AV experienced multiple unwitnessed falls from December 29, 2022, and February 23, 2023, which resulted in repeated unreasonable discomfort, including a laceration. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
