Findings: Alleged Victim (AV) is care planned to be in a two (2) hour check by staff. On or about February 20, 2023, AV was discovered to be missing at around 11:30 am by staff. After being discovered missing, staff searched for AV but claimed they became sidetracked with other tasks and put the search on hold. AV was later found by a family member at around 4 pm, which by this time there should have been approximately two 2-hour safety checks conducted on AV. When AV was found, he/she was found on the floor with redness on his/her head and knees as well as a laceration to his/her pinky finger. The facility failed to follow the care plan for AV around the 2-hour safety checks, which is a violation of resident rights, is neglect of care and constitutes abuse.
