Findings: Alleged Victim (AV) relies on the facility for his/her care. On or about February 26, 2023, AV had a fall and as a result, he/she was sent to the hospital and sustained a skin tear on his/her upper right arm and a large bruise, a laceration on his/her forehead, and a skin tear on AV’s left elbow. AV has had approximately three (3) other falls within the prior month. The facility failed to provide a safe environment for AV, which is a violation of resident rights, is neglect of care and constitutes abuse.
Sanction: RCFCP23-00978 $500.00 fine assessed
