Findings: According to documentation, the Alleged perpetrator 2 (AP2) was both physically and verbally aggressive toward the Alleged Victim (AV) while providing incontinence care, even after AV asked AP2 not to stop, causing unreasonable discomfort and a loss of personal dignity. AP2's actions in this case have been determined to be inconclusive for abuse. The facility failed to protect AV from abuse, after multiple staff complained of AP2's treatment toward residents, the failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
