Findings: Witness #1 (W1) has a history of aggressive behaviors towards other residents and members of the care team. From approximately February 22 to March 3, 2023, W1 was involved in approximately (7) physical altercations. On or about March 7, 2023, an unwitnessed physical resident-to-resident altercation took place between Alleged Victim (AV) and W1. W1 was the aggressor and pushed AV down, causing lacerations and abrasion to h/h arm. The facility failed to appropriately care plan and implement reasonable interventions to address W1’s increasing and ongoing aggressive behaviors, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP23-00995 $375.00 fine assessed
