Findings: The facility failed to properly care plan for the Alleged Victim (AV) by implementing interventions to mitigate repeated falls or injury. The failure resulted in the AV experiencing three falls in approximately one week, one fall caused the AV to hit their head causing unreasonable discomfort and required monitoring, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
