Findings: The facility failed to follow the Alleged Victim’s (AV) care plan. AV’s care plan states AV needs to be transferred with a gait belt. On or about March 08, 2023, AV sustained an abrasion to h/h left knee, due to being transferred without the gait belt. The failure resulted in AV experiencing pain and unreasonable discomfort, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
