Findings: Alleged Victim (AV) relies on the facility for his/her care. When AV first moved to the facility in December 2022, AV was able to ambulate independently with a cane. Staff were aware that AV was no longer able to use a cane and was ambulating by crawling on his/her hands and knees. On or about February 1, 2023, AV was found on the floor of their room with red marks on his/her right forehead. On or about February 5, 2023, AV was found on the floor of their room, AV was sent to the hospital due to his/her speech being slurred and slow. On or about February 6, 2023, AV had a fall in the dining room and complained of pain to the right side of their face. On or about February 15, 2023, AV was sent to the ER for sustaining a sprained ankle. The facility failed to provide oversight and monitor change of condition for AV regarding his/her ambulation, which is a violation of resident rights, is neglect of care and constitutes abuse.
