Findings: The facility failed to follow the care plan by ensuring the Alleged Victim’s (AV) walker was oriented in the appropriate direction before the AV used it. The failure resulted in the AV falling and suffering a fracture which required surgical intervention. The AV is not expected to return to baseline for ambulation and will require a wheelchair for future ambulation, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
