Findings: According to the documentation, the Alleged Perpetrator (AP2) failed to follow the Alleged Victim’s (AV) care plan when he/she shows signs of resisting incontinence care. The failure resulted in the AP2 holding AV’s wrists behind his/her back to complete cares, which is a violation of resident rights and is considered physical abuse. The facility failed to follow the care plan which is a violation of Oregon Administrative rules.
