Findings: The Alleged Victim (AV) relies on the facility for his/her care. According to documentation, on or about March07, 2023, the Alleged Perpetrator 2 (AP2) was asked to put AV’s card inside the bag of their walker. AP2 never returned with the card, resulting in AV having suspicious activity on h/h card. AP2 is responsible for financial exploitation, which constitutes abuse. The facility failed to protect AV from financial exploitation which is a violation of Oregon Administrative Rules.
