Findings: Witness 1 (W1) has a known history of behaviors. W1 has been in prior resident-to-resident altercations with Alleged Victim (AV). On or about March 19, 2023, W1 rammed into AV’s scooter with their own. AV then went to report incident to staff and as AV was reporting the incident, W1 came and got in front of AV’s scooter and began to yell to AV to “shut up” and that W1 was going to “rip AV’s head off”. The facility failed to provide a safe environment for AV, which puts AV in risk of harm, which is a violation of resident rights, is neglect of care and constitutes abuse.
Sanction: ALFCP23-00722 $375.00 fine assessed
