Findings: Alleged Victim (AV) and Witness 1 (W1) have history of altercations towards residents, and each other. W1 has history of going into other resident rooms and gets angry if anyone accuses h/h of stealing. AV service plan has interventions for redirecting that is not successful, and W1 has no interventions outlined for altercations. On or about March 18, 2023, AV accused W1 of stealing something from h/h and a physical altercation occurred resulting in AV sustaining a skin tear on h/h left hand and a scraped ankle. The facility failed to appropriately care plan and provided interventions to mitigate AV and W1 from having physical altercations which is a violation of resident rights, is considered neglect of care and constitutes abuse.
