Findings: Alleged Victim’s (AV’s) care plan states he/she is a high fall risk and would try to get up and forgets he/she cannot ambulate very well without his/her wheelchair. On or about March 16, 2023, AV was found on the floor of his/her room by a non-staff member. AV was found near his/her bed. This has not been the first occasion in which AV has been found on the floor. AV was previously found on the floor in their apartment on or about December 15, 2022, putting AV in risk of harm. The facility failed to follow the care plan regarding AV’s high fall risk, which is a violation of resident rights, is neglect of care and constitutes abuse.
