Findings: The facility failed to implement interventions and appropriately care plan related to the Alleged Victim's (AV) fall history. On or about March 19, 2023, AV suffered a fall that resulted in a bruise on his/her upper left arm and a laceration on his/her right upper arm. Based on facility documentation and interviews, AV had a previous fall on February 25, 2023 and facility failed to develop sufficient interventions mitigate risks to AV. The facility's failure to properly care plan for AV's fall risk caused unreasonable discomfort which is a violation of resident rights, is considered neglect of care and constitutes abuse.
