Findings: The Alleged Victim (AV) and Witness #1 (W1) have history of resident-to-resident altercations with other residents and each other. W1 has history of using h/h cane as a weapon and hitting walls and doors. On or about March 24, 2023, AV and W1 were involved in a Resident-to-Resident altercation. AV was walking away from dinning room table when W1 hit AV in the hip with h/h cane. The facility failed to appropriately care plan and implement reasonable interventions to address AV and W1 ongoing resident to resident altercations, and W1 use of h/h cane as a weapon which is violation of resident rights, is considered neglect of care and constitutes abuse.
