Findings: Witness 1 (W1) has a history of physical aggression toward other residents of the facility. On or about March 25, 2023, W1 and Alleged Victim (AV) were involved in a resident-to-resident altercation resulting in W1 striking at AV. W1 and AV were involved in prior altercations on or about March 3rd and 14th, 2023. The facility failed to provide a home like environment for AV, resulting in three resident-to-resident altercations within a month putting AV a risk of harm, which is a violation of resident rights, is neglect of care and constitutes abuse.
Sanction: RCFCP23-00989 $500.00 fine assessed
