Findings: According to the documentation, the Alleged Perpetrator 2 (AP2) failed to protect the Alleged Victim (AV) from physical abuse. The failure resulted in the AP2 holding the AV’s hands down, across their lap causing the AV to become angry and start yelling at AP2. AP2 in return yelled back at the AV causing physical and emotional distress, which is a violation of resident rights and is considered physical abuse. The facility failed to protect the AV from physical abuse which is a violation of Oregon Administrative Rules.
