Findings: According to the documentation, the facility failed to follow the Alleged Victim’s (AV) service plan by leaving them alone in their room while in their wheelchair. The failure resulted in the AV to attempt to self-transfer causing the AV to fall and fracture their hip, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
