Findings: Alleged Victim (AV) has a known history of elopement. AV has traveled to other cities without notifying staff, putting AV in risk of harm. On or about April 2, 2023, AV left the facility before 7am, prior to leaving, care staff noticed that AV was not wearing clothing appropriate for the weather. At 9:15pm on or about April 2, 2023, care staff noticed that AV was not in his/her room. The last time AV was seen prior to this was at 8pm. The facility was searching, and AV was not located. At 11pm, care staff notified police and an officer responded by gathering information about AV. at 12:13am on April 3, 2023, hospital staff called the facility and reported that AV had been bit by a motor vehicle and as a result, had sustained a rib fracture and skull fracture. The facility failed to provide a safe environment for AV, which is a violation of resident rights, is neglect of care and constitutes abuse.
