Findings: Alleged Victim (AV) is scheduled to be taken to the bathroom every two (2) hours and AV needs hands on assistance when going to the bathroom. On or about March 30, 2023, Alleged Perpetrator 2 (AP2) was scheduled to work NOC shift from 10pm to 6:30am; AP2 went home at 3am on March 31, 2023 and had not done toilet or safety checks on any of his/her residents. AP2 failed to provide basic care and services necessary to maintain the health and safety of AV, by not toileting AV every two (2) hours as care planned, which is neglect of care and constitutes abuse. The facility failed to assure the care plan was followed, which is a violation of Oregon Administrative Rules.
