Findings: The Alleged Victim (AV) relies on the facility to manage his/her medications. On or about April 4, 2023, it was reported that AV missed his/her required medication. During the investigation, it was discovered AV missed several required medications on multiple occasions. According to documentation and witnesses, the facility has out of AV's medication multiple times and AV experienced unreasonable discomfort, including withdrawal symptoms. The facility failed to provide a safe medication administration system to ensure AV’s medication was available to be administered as ordered. The failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
