Findings: On or about April 5, 2023, the Alleged Perpetrator #2 (AP2), placed a wheelchair and pillow against the Alleged Victim's (AV) bed in an attempt to prevent them from falling out of bed. AP2 did not follow state guidelines for resident rights/treatment and no restraints, which is a violation of resident rights, is considered neglect of care and constitutes abuse. The facility failed to ensure the residents rights/treatment was being followed, which is a violation of Oregon Administrative rules.
