Findings: The Alleged Victim (AV) relies on the facility to provide a safe environment. According to an investigation, Witness 1 (W1) was care planned on a trial-basis for a self-administering medication. W1 had been showing a decline in the ability to understand medication instructions, and on or about April 8, 2023, W1 took several medications and went into AV’s room with no pants on, which resulted in unreasonable discomfort and a loss of personal dignity. The facility failed to provide a safe environment, which is a violation of resident rights, is neglect of care and constitutes abuse.
