Findings: On or about April 15, 2023, the Alleged Victim (AV) was having behaviors by flooding his/her bathroom and pulling items from the walls. AV was known to have behaviors, but no care plan was put into place to ensure staff knew how to assist when AV was having behaviors. The facility failed to properly care plan and have proper interventions in place to reduce the risk of harm to AV. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP23-01060 $500.00 fine assessed
