Findings: The facility failed to provide a safe medication administration system to ensure the AV's medication was stopped as ordered and failed to provide services according to his/her incontinence needs. The failures resulted in a delay in delay of AV's surgery for failure to stop his/her medication; AV being not being showered according to his/her care plan and his/her room smelling of incontinence products, which are violations of AV's rights, is considered neglect of care and constitutes abuse.
