Findings: Alleged Victim (AV) and Witness 1 (W1) have history of verbal altercations and aggressive behavior with other residents and each other. On or about April 24, 2023, AV and W1 behaviors escalated from verbal incident to physical incident resulting in injuries. AV sustained swelling to h/h right eye, skin tears to right bicep, and left hand near pinkie finger. W1 sustained skin tear to h/h left hand, and index finger near knuckle. The facility failed to appropriately adjust care plans and implement reasonable interventions to address AVs and W1s continued aggressive behaviors, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
