Findings: According to the documentation, the facility failed to follow the care plan for Witness 1 (W1) to supervise W1 to ensure they do not wander into other resident rooms and redirect them if they do. The failure resulted in W1 wandering into the Alleged Victim’s (AV) room causing a resident-to-resident altercation and W1 slapping the AV in the face, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
