Findings: The Alleged Victim (AV) resides in the memory care and Witness #1 (W1) resides in the assisted living. W1 is known to have inappropriate behaviors with residents and his/her visits to the memory care are to be in the common area only and staff are to keep W1 in sight. On or about May 3, 2023, W1 was let into the memory care unit to visit another resident and was found in AV's room with his/her hand inside AV's shirt. The facility's failure to ensure AV's safety is a violation of resident rights, is considered neglect of care and constitutes abuse.
