Findings: On or about May 2, 2023, Witness 1 (W1) hit Alleged Victim (AV) in the head resulting in AV yelling out at W1. Based on facility documentation and interviews, the pharmacologic and non-pharmacological interventions in place were not successful for preventing W1's aggression. During the month of April 2023, W1 engaged in six other resident-to-resident altercations. The facility failed to implement appropriate interventions and appropriately monitor W1 according to his/her known behavior and prior altercations, causing unreasonable discomfort to the AV, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
