Findings: On or about April 30, 2023, the facility failed to ensure adequate staff were available to administer medications to Alleged Victim (AV) as ordered. As a result of facility's failure to provide a safe medication administration system, AV missed a single administration of his/her medications which is a violation of Oregon Administrative Rules. Alleged Perpetrator 2 (AP2) allegedly neglected AV. An investigation determined no wrongdoing by AP2.
