Findings: The Alleged Victim has a history of falls and was care planned as a high fall risk. The facility failed to provide appropriate services according to Alleged Victim (AV)'s needs, number of falls with lack of appropriate interventions to mitigate fall risk. Based on an investigation, AV experienced approximately six falls from approximately January 17, 2022, and May 4, 2023, which resulted in repeated unreasonable discomfort, including a skin tear and a hip fracture. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
