Findings: The Alleged Victim (AV) is a known fall risk. AV requires a two person assist with a hoyer lift. AV has experienced 6 falls between February 14, 2023 and March 8, 2023, with and without injury. On or about March 18, 2023, AV was found on the ground after experiencing an unwitnessed fall in his/her residence. AV expressed pain in his/her knee, bruising was observed on his/her knee and back. The facility's failure to ensure proper and effective interventions were in place to assure AV's safety is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP23-01069 $1500.00 fine assessed
