Findings: The Alleged Victim (AV) is a known fall risk. AV requires a two person assist with a hoyer lift. AV has experienced multiple falls, with and without injury. On or about April 8, 2023, AV was found on the ground after experiencing an unwitnessed fall in his/her residence. AV expressed pain in his/her rib area. The facility's failure to ensure proper and effective interventions were in place to assure AV's safety is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP23-01069 $1500.00 fine assessed
