Findings: On or about May 1, 2023, the Alleged Victim (AV) was found sitting in urine and feces, with urine soaked through his/her chair and leaking onto the floor. On or about May 6, 2023, AV was again found in his/her room, soaked through his/her incontinence brief with his/her recliner soaked, and urine dripping off him/her when he/she stood up. According to documentation, the facility was not changing AV's incontinence brief regularly, causing AV to have have redness and a sore on his/her buttocks, leading to unreasonable discomfort. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
