Findings: Alleged Victim (AV) relies on the facility for his/her care. On or about January 26, 2023, AV was found on the ground, face up on his/her room. On or about March 7, 2023, at 6:53am, AV slid off their bed and was found sitting on the floor wearing a shirt and no pants. On that same day, at 11:40pm, a staff received a fall alert, AV was found on the ground, face up not wearing shoes or socks. On or about March 14, 2023, at 3am, AV slipped out of bed and fell to the ground. On or about March 21, 2023, at 12:24pm, AV was found sitting on the floor next to his/her bed. As a result, AV suffered a skin tear on his/her left elbow. The facility failed to provide a safe environment for AV, putting AV in risk of harm, which is a violation of resident rights, is neglect of care and constitutes abuse.
