Findings: According to an investigation, the Alleged Victim (AV) and Witness 2 (W2) were involved in a resident-to-resident altercation on or about May 9, 2023, which resulted in a temporary service plan stating that AV and W2 are not to be around each other unsupervised. On or about May 10, 2023, AV was found on the floor with W2 hovering over the AV and AV complaining of pain in his/her left elbow and wrist. The facility failed to follow the care plan, which is a violation of resident rights, is neglect of care which constitutes abuse.
Sanction: ALFCP23-00505 $188.00 fine assessed
