Findings: The Alleged Victim (AV) and Witness 1 (W1) have a history of verbal and physical altercations. The facility failed to provide a safe environment to the Alleged Victim (AV). According to an investigation, on or about May 11, 2023, AV and W1 had a resident-to-resident physical altercation, which resulted in AV sustaining skin tears on their hand and forearm. The facility’s failure is a violation of resident rights, is neglect of care which constitutes abuse.
