Findings: The facility failed to implement interventions and appropriately care plan regarding Alleged Victim’s (AV) known fall risk. AV had falls on or about April 6, 2023, and April 29, 2023, which are both documented to have resulted in in injury. AV was not sent out to the hospital until May 6, 2023, where AV was diagnosed with a fractured vertebrae in h/h back. The facility failed to implement interventions to mitigate AV’s increasing fall risk, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP23-01301 $1500.00 fine assessed
