Findings: The Alleged Victim (AV) relies on the facility for his/her care. AV is known to experience confusion and has poor safety awareness. According to an investigation, on or about May 12, 2023, staff found an empty bottle of sanitizer fluid on AV’s dresser that was determined to have come from an unsecured location on medication cart. AV experienced throat and, stomach pain and a raspy throat. The facility failed to provide a safe environment, which is a violation of resident rights, is neglect of care which constitutes abuse.
